EECP Is a Non-Surgical Option for Cardiac Veterans

Enhanced External Counterpulsation (EECP) is a non-invasive, outpatient treatment that increases blood flow to the heart by inflating and deflating cuffs on the legs in sync with the heartbeat. It is FDA-cleared for chronic stable angina and heart failure โ€” two conditions that affect veterans at significantly higher rates than the general population.

Veterans who have exhausted medications, are not candidates for stents or bypass surgery, or are looking for a non-surgical alternative should ask their VA cardiologist about EECP. Medicare covers EECP for eligible beneficiaries, and VA Community Care may authorize treatment at community providers when VA facilities cannot provide timely access.

Agent Orange-related ischemic heart disease is a VA-recognized presumptive condition, and veterans with this diagnosis may qualify for EECP as part of their service-connected care.

VA facilities that offer EECP can provide it directly to enrolled veterans. Coverage is determined by the treating VA physician and the veteran's service-connected or non-service-connected eligibility.

Medicare Part B

Eligible conditions: Class IIIโ€“IV chronic stable angina refractory to medical therapy

Medicare covers EECP under CPT code 92971 for eligible beneficiaries. Veterans who are Medicare-enrolled can use Medicare at non-VA EECP providers, which is often faster than waiting for VA scheduling.

Veterans can use both VA benefits and Medicare simultaneously. If your VA facility does not offer EECP, you may be able to use Medicare at a community EECP provider through the VA Community Care program.

Chest pain from reduced blood flow to the heart is the primary FDA-cleared indication for EECP. Veterans with refractory angina โ€” pain that persists despite medications โ€” are ideal candidates. Medicare covers EECP specifically for this indication.

EECP is FDA-cleared for heart failure and has been shown to improve exercise tolerance and quality of life. Veterans with reduced ejection fraction who are not surgical candidates may benefit significantly.

PAD is common in veterans, particularly those with diabetes or a history of smoking. EECP improves peripheral circulation and has been used off-label to reduce claudication (leg pain during walking) and improve wound healing.

Veterans with service-related or post-service diabetes frequently develop neuropathy. EECP's mechanism of improving microvascular circulation has shown benefit for diabetic neuropathy symptoms in clinical use.

How to Access EECP as a Veteran

Talk to your VA cardiologist

Ask your VA cardiologist or primary care provider whether EECP is appropriate for your condition. Bring a list of your current medications and describe your symptoms โ€” particularly if you have refractory angina or heart failure.

Ask if your VA facility offers EECP

Not all VA medical centers have EECP equipment. If yours does not, ask about a referral through the VA Community Care program, which can authorize treatment at a community EECP provider near you.

If you are enrolled in Medicare Part B, you may be able to access EECP at any Medicare-participating community provider without waiting for VA authorization. Most veterans over 65 are dual-eligible for VA and Medicare.
